That last point matters more than it may initially seem.

Flange Size Matters – A Lot
One of the biggest improvements we have seen in breast-pump education over the past few years is a much greater focus on correct flange fit.
Your flange size is based on your nipple dimensions, not your breast or bra size. Someone with an A cup and someone with a G cup could therefore require exactly the same flange.
GEM already gives users more options than many pumps straight out of the box, with 18 mm, 20 mm and 22 mm silicone inserts as well as its 24 mm breast shield. Smaller inserts are also available separately at Fed is Best for mothers who need them.
And newer research is challenging the old assumption that a nipple needs a generous amount of space around it inside the flange tunnel.
A 2025 pilot study published in the Journal of Human Lactation compared traditional flange sizing with a newer, individualised smaller-fit approach. Participants reported significantly better comfort with the smaller fit and expressed, on average, more milk per pumping session.
However, the important takeaway is not simply “smaller is always better.”
The researchers specifically recommend an individual fitting process. Their method starts with nipple-tip measurement and then trials nearby sizes while considering comfort and milk flow. Different bodies may respond differently.
So, if pumping hurts, your nipple rubs excessively, too much breast tissue is being pulled into the tunnel, or your output seems unexpectedly low, flange fit is one of the first things worth reassessing.
It is also completely possible to need a different flange size on each breast.
Can You Use GEM if You Have Larger Breasts?
Yes. Breast size itself does not determine flange size.
What matters most is correctly centring the nipple within the breast shield or flange insert and securing the pump inside a supportive bra.
GEM is designed for use with a standard nursing bra and includes a bra adjuster to create additional room when required. The bra should hold the pump securely without compressing it excessively.
How Do GEM’s Four Pumping Modes Work?
GEM offers four modes rather than simply one stimulation setting and one expression setting.
Massage and Stimulation modes are designed for the beginning of the session and encouraging milk flow, while Expression and Ultra Expression offer different pumping patterns once milk is flowing.
Each mode has nine intensity settings.
The important thing here is that stronger suction does not automatically mean more milk.
The goal should be effective milk removal at a vacuum level that remains comfortable. Excessively high suction can cause discomfort, and pumping should not hurt.
That is why having 9 levels within each GEM mode is useful: it allows you to experiment rather than assuming that the maximum setting is automatically the best setting.
Is GEM a “Hospital-Grade” Breast Pump?
This is where we want to make an important distinction.
You will often see the phrase hospital-grade breast pump used across the breast-pump industry. However, there is no universally recognised definition for the term.
The US FDA, which regulates breast pumps as medical devices in the United States, specifically states that it does not recognise “hospital grade” as a defined category and that manufacturers may use the term differently.
For that reason, we think GEM’s actual specifications are far more useful than a label.
GEM offers adjustable vacuum from 100 to 270 mmHg, four pumping modes, nine levels within each mode and a closed system that uses a diaphragm to create a barrier between expressed milk and the motor.
Those are measurable features you can actually compare when choosing a pump.
Can GEM Be Your Only Breast Pump?
It can be.
GEM is a fully functional electric breast pump, and many mothers choose wearables because they allow them to express more easily during work, travel or a busy day.
But there is an important point we make about every breast pump: how well someone responds to a particular pump is individual.
A high maximum suction specification does not guarantee that one pump will remove milk equally effectively for every person.
If you are exclusively pumping, establishing your milk supply in the early postpartum weeks, expressing for a premature or medically fragile baby, experiencing persistently low output or dealing with pain, we recommend getting individual advice from an IBCLC or other appropriately qualified lactation professional.
Pumping itself can absolutely be used to establish and maintain milk production when direct feeding at the breast is not possible or does not form part of your feeding plan.
How Long Can You Pump With GEM?
GEM automatically switches off after 30 minutes.
According to GEM, a full battery charge takes approximately three hours and provides around 2.5 hours of pumping time, although actual runtime can vary according to the mode and intensity used.
It charges via the supplied USB-C cable. GEM recommends charging from a standard 5V/1A or 5V/2A wall adapter.
Is GEM Really Quiet?
GEM’s specified operating sound level is below 50 dB.
That does not mean completely silent. Like any electric motor, you can hear it in a very quiet environment.
What it does mean is that GEM has been designed with discreet pumping in mind, alongside its compact in-bra shape.
We prefer that description to promising that “nobody will hear it”, because real-life environments differ.
A Closed System – But Still Assemble It Carefully
GEM is a closed-system pump.
Its silicone diaphragm forms a barrier intended to stop milk from entering the motor unit. The motor therefore does not need to be washed or submerged.
Closed system does not, however, mean a pump is impossible to leak.
Correct assembly matters. Worn duckbill valves or O-rings, an incorrectly seated bottle or diaphragm, or damage to a component can affect suction and cause leaking.
This is also why replacement parts matter.
One of GEM’s Biggest Advantages: Parts Are Replaceable
Breast pumps contain consumable parts.
Silicone valves, seals and diaphragms flex thousands of times during regular pumping and gradually wear. When they do, suction and pump performance can change even though there is absolutely nothing wrong with the motor.

We see that as a major advantage.
A breast pump should not become useless because a small silicone component needs replacing.

What About Cleaning and Sterilising?
GEM’s milk-contact parts should be disassembled and cleaned according to the manufacturer’s instructions after pumping.
GEM allows specific components to be sanitised using boiling, a compatible standalone steam steriliser or UV sterilisation within its specified temperature limits. It specifically advises against microwave steam sterilisers and chemical sterilising solutions for its parts.
Always follow the instructions for the pump you actually own rather than assuming that a sterilising method suitable for one brand is suitable for another.
Is the Gem Breast Pump Hospital-Grade?
The term “hospital-grade” can vary depending on how it’s defined. The Gem Breast Pump meets many of the criteria often associated with hospital-grade pumps:
